Boone County Schools Health Services is dedicated to ensuring that all children reach their full potential by collaborating with families, staff and community partners. Our staff of nurses and health support staff follow CDC and Boone County Health Department guidelines to provide families with knowledge concerning immunization requirements, preventative health services, and other medical screenings. By working together we can assist families in meeting each student’s physical and emotional needs; ensuring success today and in the future.

Vaccination Records

Parents/Guardians can now access their child's vaccination records, print a school immunization certificate and download a digital copy of their COVID-19 vaccination record through the secure Kentucky Immunization Registry (KYIR) Public Portal.   Please see the link below for information on how to access this information.  https://www.chfs.ky.gov/agencies/dph/dehp/idb/Pages/kyirportal.aspx  

 

Reporting Illnesses

Per Kentucky Regulations, (KRS 158.160), "If any student is known or suspected to have or be infected with a communicable disease or condition for which a reasonable probability for transmission exists in a school setting, the superintendent of the district may order the student excluded from school."  Parents and guardians are obligated to notify their student's school immediately if their student is known or suspected of having a communicable disease.  Refer to this Table of Reportable Diseases.  Contact your school's nurse with any questions.
