Please complete THIS FORM to request the Kentucky School Compliance or No Pass / No Drive form for student driver's license permits.

To qualify for the School Compliance Form, you must:

1. Be between 15 and 17 years old
   (Eligibility based on your date of birth)

2. Have fewer than 9 unexcused absences
   (Based on attendance from the previous semester:
   August–December or January–May)

3. Be passing at least 4 out of 6 academic classes
   (Also determined by grades from the previous semester)

Please allow 24 - 48 hours to process your request. Completed forms can be picked up in the front office.
